Schedule here via our patient portal. We’ll then send you our digital intake forms. These must be completed 48 hours before your nutritionist appointment. If you are using insurance, please also upload your insurance details (there is a form for this as well).
Hoping to use insurance?
We accept Blue Cross Blue Shield of NC, Cigna, Medicare, Aetna, First Carolina Care, United Healthcare, MedCost, Medicaid of NC, and Tricare. Nutrition therapy coverage varies by company and plan. As a first step, grab our insurance script HERE and call your health plan to verify your nutrition coverage. Please send us your completed script to info@empowerednutrition.health .
